online during the reporting periods described in the adjacent table.

Lost Card?
If you have lost your card, to the best of your recollection record the
dates and locations of EACH of your fishing trips (even if you didn’t catch any steelhead on a given trip),
catches, and releases (wild and hatchery steelhead) and return that information.
